What is the relationship between Preadmission Screening and Resident Review (PASRR) and the Minimum Data Set (MDS) 3.0?

PASRR is guided by the Code of Federal Regulations (CFR) at 42 CFR Part 483 Subpart C that require: (1) all individuals being considered for admission to a Medicaid-certified nursing facility (NF) be screened prior to admission to determine if the person has, or is suspected of having, a mental illness (MI), intellectual disability (ID), or related condition (RC); and (2) all NF residents experiencing a significant change in mental or physical condition are to be reviewed to determine if the change is connected to a MI, ID or RC. The PASRR process is intended to assure that NF placement is, or continues to be, appropriate and that any individual with a MI, ID, or RC receives services that may be needed to address their unique needs. The state Medicaid agency (SMA), the state mental health authority (SMHA) and the state intellectual disability authority (SIDA) are responsible for the administration of PASRR.


It is important that states understand that CMS guidance to NFs in the Long-Term Care Facility Resident Assessment Instrument 3.0 User’s Manual on actions to take when there is a significant change in condition addresses individuals who have previously been identified by PASRR and individuals who may not have been identified as having a PASRR condition at the time of admission.
