Indiana PASRR Program Improvements - Part II

The September Indiana PTAC webinar is Part II of the broader state initiative to improve its PASRR process. A key feature of their improvement effort is the recognition that PASRR shares many goals associated with the No Wrong Door initiative in the state. The webinar will include discussion on how these two distinct initiatives share common values and goals. The webinar will help states understand the lessons Indiana learned in connecting the NWD and PASRR initiatives.

This webinar highlights how the state of Indiana, with leadership from the Division of Aging (DA), has significantly transformed their PASRR system through a highly collaborative process. From 2014 through 2016, the DA worked with a range of stakeholders to build support for, and ownership of, the new PASRR system, including the state’s Area Agencies on Aging, nursing facilities, the Indiana Hospital Association, advocacy organizations, and other divisions within the Family and Social Services Administration. The stakeholders agreed their objective was “to provide a person-centered PASRR system that effectively and efficiently identifies the most appropriate services and settings.”
Telehealth and PASRR: Opportunities for Preadmission Screening and Resident Review

This webinar will provide an overview of telehealth and the PASRR program and discusses the potential applications of telehealth to address barriers in PASRR. Although telehealth offers the opportunity to improve program efficiency and effectiveness, PASRR authorities must balance state-specific barriers, available resources, operational considerations, regulatory requirements, clinical quality, and budgetary limitations.
The 30th Anniversary of PASRR

A look at PASRR’s past and its future. Since the passage of the Omnibus Budget Reconciliation Act of 1987, over thirty years ago, PASRR has become a critical part of the health care continuum and a driver of excellence in person-centered practices and individualized plans of care. This webinar will look back at pivotal moments in PASRR’s history, look at how PASRR implementation has changed over time, and consider important changes going forward. From practices to lessons learned to industry standards, states have played a major role in PASRR’s continuing success.
MDS State Profiles

Findings of the 2014 and 2015 PTAC national reports point to issues in properly identifying individuals with PASRR conditions residing in nursing facilities. The webinar will describe the importance of using MDS data as a tool in investigating the reasons behind this issue and defining the MDS State Profiles distributed to states in late April 2018. The speakers will discuss the various hypotheses associated with the findings of the MDS data and share practical applications for using the MDS State Profiles in states. The webinar will also highlight the ways in which PTAC can support states’ drill-down of the data and strategies for making PASRR program changes to improve the MDS results.
Discharge Planning and Transitions into the Community

A key objective of this webinar is to clarify the boundary conditions of transition services offered as part of the NF benefit. For example, it seems clear that transition services can last an indefinite amount of time—whether it takes someone 6 months or a year or 2 years to transition doesn't matter, in the sense that there is no cutoff point for those services (unlike with waivers and MFP). It's also clear that transition services in the NF benefit cannot cover payments that MFP can cover, such as a security deposit. However, it may be possible to buy certain goods for NF residents if doing so would help them prepare to transition successfully. For instance, if an individual's transition plan requires that he or she learn how to use a microwave, to heat foods at home, a Specialized Service within the NF benefit could enable him or her to purchase a microwave which they could then take with them. This webinar will answer questions and provide guidance in situations that are not immediately clear.

Preadmission Screening and Resident Review (PASRR) is a federal requirement to help ensure that individuals are not inappropriately placed in nursing homes for long term care. This webinar will provide an overview of the basics of PASRR. An in-depth exploration of the PASRR regulation, from the origin of the regulation through its evolution to the present, will be covered. The fundamentals of the PASRR regulation will be broken down. Promising practices, common challenges, and frequently asked questions will be addressed.

This webinar will highlight key findings from the 2017 National Report. The results present a snapshot of PASRR programs as of September 2017, with focus on our analysis of MDS data, and on our analysis of data that states collect for quality monitoring and quality improvement (QM/QI).
Strategies in Intellectual and Developmental Disabilities (IDD) and Mental Illness (MI) PASRR Program Collaboration

This presentation will provide strategies for integrating the PASRR evaluation process, cross-consultation, quality monitoring, and service delivery to support person-centered care for individuals with co-occurring disorders. Examples of state procedures will be used to illustrate various approaches in integrated PASRR systems.
